Miriam Crennan, a Senior Occupational Therapist with the Children's Disability Network Team in Kilkenny, has developed a program aimed at addressing this issue head-on. Crennan explains that she works with children facing a wide range of complex needs, many of whom grapple with sensory processing challenges that hinder their ability to brush their teeth effectively. This can result in compromised dental health, underscoring the importance of early intervention. She highlights several reasons why children might find toothbrushing overwhelming. The bathroom environment itself, with its potential darkness, brightness, noise, or cold temperature, could be a source of anxiety. Visually, some children may dislike the appearance of toothpaste, while others struggle to interpret the sensations of the toothbrush bristles against their teeth, the taste of the toothpaste, or the texture of the brush. Past traumatic experiences, such as a fall resulting in dental injury, could also lead to a fear of toothbrushing, as the act becomes associated with pain and the dentist. For parents facing these challenges, Crennan advocates for a gradual and step-by-step approach. Creating a warm and inviting bathroom environment, not too bright, with a stool to allow the child to see the mirror and understand what's happening, can be beneficial. Providing visual feedback through a hand mirror can empower the child, making them feel more in control. If a child has extreme tactile sensitivity, starting with touch away from the face, gradually escalating from the feet towards the mouth, can help desensitize them. Using a gum massager before introducing a toothbrush can further ease their transition. Crennan emphasizes the importance of selecting the right toothbrush and toothpaste. Offering children choices among different toothbrush types, such as a Dr. Barman Toothbrush with three sides for faster cleaning, can be helpful. For children sensitive to taste, unflavored toothpaste might be preferable. Some children even respond well to electric toothbrushes if they are not bothered by the noise. Choosing a calm moment for toothbrushing, rather than bedtime, is crucial. Ultimately, the program aims to enhance both the child-parent relationship and create a sense of community among parents facing similar challenges. Crennan's work has been recognized with the Eva Duggan award at the Association of Occupational Therapists Ireland Conference, providing her with a €1,000 bursary to further develop her program. She plans to initially reach 100 children in her area and hopes to record webinars to train other Occupational Therapists in delivering similar programs. She also envisions the program benefiting adults with learning disabilities
