A 76-year-old man was sent home to die after he was transported to University Hospital Limerick by ambulance and subsequently discharged.

The Cuddihy family’s grief was compounded by the news of another tragic death due to sepsis, that of 16-year-old Aoife Johnston. Michael’s daughter Anne said: “Poor Aoife died December 2022. My dad died November 2023. My dad died from sepsis too, and I just don't understand how after the death of a 16-year-old girl, they cannot go all out to ensure that this does not happen to somebody else, whatever age they are. They clearly didn't learn from that.
After receiving the post-mortem results, the family requested a meeting with senior management at UHL. The meeting was arranged for January of this year, over a year after the death of Aoife Johnston. Senior medical staff confessed that if they could "rewind the clock," Michael’s care would have been different.
