Joanne Gallacher met James Kennedy while receiving treatment for mental health issues in Hairmyres Hospital in East Kilbride, Lanarkshire.
A man who stabbed his girlfriend to death told medical staff he believed she was planning to harm him, an inquiry heard.
A note from the meeting said: "James self-referred with paranoid ideas that his friend Joanne was making plans to harm him. James described as being able to hear Joanne's thoughts and that she planned to harm him when he was asleep. He was deemed as posing an 'immediate risk' to himself and others and was a 'medium risk' of suicide but had no plans to kill himself.
