He was acting 'bizarrely' and 'psychotic' before his death
An inmate with a history of suicide attempts and who warned he might hurt himself took his own life at HMP Preston after health care workers chose not to monitor him.
According to the investigation, he is the third person to die at the Category B prison since May 2020 and one of the others was also self-inflicted. However, an investigation into that prisoner's death has not yet been completed. On an assessment a day later, he said that he did “want to be on this earth much longer” and that he “wouldn’t be bothered if he fell asleep and didn’t wake up”. However, when asked, Mr Smith said he did not have active plans to harm himself.
