'Mad Chainsaw Mode' makes the demo more difficult, both in a general way and thanks to the inclusion of Flaming Chainsaw Man. He's like the normal Chainsaw Man, but his weapon is on fire. And he will probably wreck you.
In order to play through this difficulty you have to be lucky enough to be offered the option when selecting a New Game. If you don't get the offer, reset the game and try again. Some people are stating that you need to beat the demo normally at least once first, although it's not necessarily the case. It's probably a good idea to play the demo first to get an idea of what you need to do, as things have changed from the original version.
