Mental health professionals ignored a family's concerns and failed to book appointments for a Preston man who took his own life.

The HTT should have made an appointment, which would have been arranged for within five days, but they failed to do so. Just two weeks after the assessment Mark was found dead at home by his mum and his family believe he might still be alive had it not been for multiple failings by the NHS trust. "There were several calls from Mr Wallbank's family, between October 2021 and January 2022, about his deterioration but there was limited evidence that the family were listened to. It was inadequate."

Mark's family asked what steps had been taken to prevent "this devastating situation from happening again". LSCFT's director of nursing and quality, Rebekah Roshan, agreed that the treatment Mark received from the trust was "very concerning". Ms Roshan said staff are now put through an additional four-day training programme which includes a full day focusing on engagement with and listening to a patient's family and friends. She added: "It's early days but the data does indicate there had been a reduction in the number of serious incidents like the one we are here discussing today."
