13-year-old girl died after just one sip of Costa drink 'due to allergy blunder'

Hannah Jacobs suffered an 'immediate reaction' as her hot chocolate was made with the wrong kind of milk

A 13-year-old girl with a severe dairy allergy died after drinking a Costa Coffee hot chocolate following a “failure to follow the processes in place to discuss allergies”, an inquest has found.
